Program Description:
The Physical Rehabilitative Care Program provides high-intensity, time-limited, interprofessional rehabilitation, with a focus on clients with neurological conditions. The Interprofessional Team consists of a Physiatrist, Family Physicians, Occupational Therapists, Physiotherapists, Registered Nurses, Registered Practical Nurses, Rehabilitation Assistants, Registered Social Workers, Registered Dietitians, Speech Language Pathologists, Indigenous Health Associates, and Mental Health resources. The program works closely with Home and Community Care Support Services, Spinal Cord Injury Ontario, and the Care Group’s Outpatient Neurology Services in order to facilitate client discharge back to the community with formal support services.
